1. Pay via credit card. Scroll to the bottom of the screen and select, “Proceed to payment.” A screen will pop up where you can securely input and submit your payment information and application.

2. Pay via check. Scroll to the bottom of the screen and select, “Print.” Mail registration form and check together in an envelope. All checks are to be made payable to the:

National Italian Invitational Golf Tournament for Charities and sent to P.O Box 510227, Milwaukee, WI 53203-0041.
